The State Board of Education requires that a permanent individual health record be maintained for your child throughout the elementary school years. This record contains information regarding your child's health history, immunization status, health screening results and other pertinent health information/observations. Particular health record questions should be directed to the nurse based at your child's school.
This form is filled out at the time you register your child for school (usually Kindergarten). Please help the school nurses stay current and informed on your child's health status/needs by completing an updated Health History form with current health information and emergency contact information for your child. If you have not recently updated this information, please download this document, complete the information, and return it to the school nurse. The information on these forms enable the nurses and staff to keep your child safe and healthy while attending school.
